Dear Venu,
Google or any other reputed companies will never ask you to fill in any forms like this.
They always makes it clear that you should not disclose your password and sensitive information to any one. The case is same with of online bank accounts or any other password oriented web services. if you do a googling regarding this matter ,you will get a lot of info about this.
Here is google's say on this http://mail.google.com/support/bin/answer.py?hl=en&answer=8253 Now I guess that's clear for you. Kindly revert if you have any doubts.
